The main choice is inner ankle, outer ankle, front of ankle or a small wrap around the joint. Each one moves differently when you walk, and each one heals with different rubbing from socks, shoes and boots.
What I check before drawing
I check visibility, the space between the lines and where shoes or socks would rub during healing. If the design needs more room, I'll explain that before the stencil stage.
The pain chart is useful because ankles can feel sharp, especially near the bone. The portfolio helps you judge line weight and scale before you send the idea.
Motifs and size
Small florals, charms, short script, fine symbols, simple Irish references, tiny blackwork and narrow ornamental bands can suit the ankle. I would simplify very tiny lettering or crowded detail. A larger design may fit better higher on the calf or leg.
